The school is responsible for completing the Secondary School Report Form or the Counselor Report Form and mailing it to each college. The Report Form must be turned in to your counselor in a timely fashion (deadlines are determined by each school) after you have filled in as much information as possible ahead of time such as your bio information and your current year classes. The counselor will fill out the school information, your ratings, and provide an evaluation. In most cases, a letter of recommendation written by your counselor describing your personal and academic characteristics is sent with this report to each school you are applying to. The deadline to submit the Report Form should be at least four weeks before they are due but as counselors are inandated with this form for all seniors applying to colleges, the sooner you turn it in, the better!
